People need to bring  

• Valid form of identification (for example, driver’s licence, proof of age card or passport)
• Medicare Card or IHI (if you have one)  

Clinic will be open: 

• Thursday 7th October – Sunday 10th October
• Between the hours of 9:30 – 5pm
